We offer members a wide range of programs designed to meet your health and wellness needs, no matter where you may be on the continuum of care. We can supply you with a comprehensive suite of effective resources and tools, providing a tailored experience built on the demands of our members.

Quarterly wellness newsletter

Be Well is a collection of news, tips and reminders designed to help you and your family make the most of your wellness benefits and live a healthy lifestyle. We email the newsletter to Baylor Scott & White Health Plan employer group members who are registered for MyBSWHealth. You can always find the current issue here.

Digital Wellness Platform (powered by WebMD®)

(For Employer Group and Individual & Family Members)

Our digital wellness platform, powered by WebMD, encourages members to be healthy, while providing personal health support for various health conditions and health risks.

To access the platform and take advantage of the activities below, log into MyBSWHealth then select Insurance & Billing > Baylor Scott & White Health Plan > Wellness in the Dashboard.

Well-Being Assessment

The well-being assessment is a simple, digital health survey that helps you take steps toward a more vibrant and healthier life.

Digital Wellness coaching

Stay engaged, track progress and accomplish well-being goals by utilizing Daily Habits. Daily Habits offers you a personalized set of activities based on your health information and personal interests.

From your health plan

Within the Digital Wellness Platform, members will be able to access additional wellness offerings available through the health plan; such as findhelp, Wellness Webinar registration and the Be Well quarterly wellness newsletter.

Wellness challenges

Join a wellness challenge for the motivation to go the extra mile. Wellness Challenges encourage sustainable, healthy habits through competition and accountability. Gamify drinking water, taking the stairs and other activities that challenge and boost your well-being.

Preventive Care

A lifetime of good health starts with quality medical care throughout your life. Thanks to the Affordable Care Act (ACA), this care is now within the reach of all Americans. ACA requires health plans to eliminate cost sharing and cover preventive care services recommended by the U.S. Preventive Services Task Force (USPSTF), guidelines specifically issued for women and adopted by Health Resources & Services Administration (HRSA), the Advisory Committee on Immunization Practices and the Bright Futures Guidelines of the Academy of Pediatrics.

The following recommendations are general guidelines for all ages and should not dictate the care you receive. Your doctor will let you know what tests and exams you may need based on your specific risk factors and overall health.

  • Guidelines may change throughout the year based on new or revised laws and/or regulations and additional guidance.
  • Services must be billed with a primary diagnosis of preventive, screening, counseling or wellness, if applicable, to qualify and other restrictions may apply.
  • Your benefits plan may contain additional preventive care benefits, exclusions and limitations. Check your plan documents for a complete description of benefits, exclusions, limitations and condition of coverage.
  • Grandfathered plans (a plan that hasn't changed since 2010) may have costs. See your plan documents to find your specific coverage and costs.
  • Talk to your healthcare provider about a proper schedule of check-up visits.

Disease Management

Disease management programs are designed to improve the health of persons with chronic conditions and reduce associated costs from avoidable complications. These goals are accomplished by identifying and treating chronic conditions more quickly and more effectively, slowing the progression of those diseases. Disease management is a system of coordinated healthcare interventions tailored to your conditions where self-care efforts can be implemented.

We have programs for our members who have asthma breathing trouble (such as bronchitis, emphysema), diabetes, heart problems and many other conditions. Our health coaches may contact you to invite you to participate in one of these programs when we see an insurance claim with one of these conditions. Disease management empowers you, working with your healthcare providers, to manage the disease and prevent complications.

  • BSWH Employee Plan members can opt in or opt out of the program by calling 844.279.7589
  • Medicare Advantage members can opt in or opt out of the program by calling 844.279.7589

Case Management

We have case managers available to help members who have been diagnosed with complicated health problems. A nurse case manager will work with members, families and the physician to create a plan to meet the member's ongoing complex care needs. Case managers advocate for members and assist them with setting goals and making a personal plan to improve their health. They also can assist with arrangements for necessary services and community supports. Case managers answer questions and provide education to help members have a better understanding of their condition and plan of care.

Healthy Pregnancy Information

As a Baylor Scott & White Health Plan member, you automatically have access to benefits designed to help you enjoy a healthy pregnancy journey. To learn more about your benefits:

  • Log into the MyBSWHealth member portal to view your benefit information.
  • Call the customer service number on the back of your ID card.

Maternity Care Management

Members enrolled in Maternity Care Management receive additional support to meet ongoing care needs, including setting goals, making a personal plan to improve their health, provide education, helpful tips and assistance during pregnancy and after delivery. There is no additional cost to members for this program. Benefits include:

  • Education and support during pregnancy and after delivery
  • Coordinated care to promote healthy pregnancy and full-term birth
  • In-home support for high-risk conditions (such as diabetes, high blood pressure and severe nausea)
  • Screening for early identification of perinatal depression
  • Newborn health education
  • Breastfeeding best practices

Maternal Mental Health Hotline

Text or call HRSA's National Maternal Mental Health Hotline to reach a trained counselor who can listen and support pregnant women and new mothers.

BSW Be Well™ with Diabetes

Get support to help improve your health, better manage your diabetes and save money. Baylor Scott & White Health Plan members who are members of Commercial Large Group (excluding HDHP) and at least 18 years old with Type 2 diabetes may participate in the program.

Advance Care Planning

Plan ahead for peace of mind.

Sometimes a serious illness or injury can leave a patient unable to communicate, and families have the burden of making difficult decisions without knowing what their loved one really wants. An advance care plan lets you make your medical wishes known ahead of time.

Community Resources - findhelp

Finding help just got easier. findhelp is a Texas-based platform created to connect all people in need with programs that serve them. You can search for local resources like medical care, food, job training, transportation and more. Organizations in your community are ready to help.
